The idea
for Families FIRST was developed and piloted for a period of years
in response to the health challenges faced by New Mexico families
during pregnancy and into early childhood. It is offered as a Medicaid-covered
service through Lovelace or Molina Salud at no cost to families.
The program has proven effective in improving the health of pregnant
women and their babies.
The SJC Family Resource
Center Families FIRST provides:
Assistance with Medicaid
applications for children and pregnant women
Care coordination
services as determined by families (Salud Managed Care Organization)
Ages and Stages Questionaire
(developmental screening for children)
Collaboration with
other community resources and services
Home visits focused
on prenatal wellness and child health
What Is Care Coordination?
Care coordination supports families in staying connected with their health
care providers and in accessing other community resources.
Who Is Eligible?
Medicaid or Medicaid
eligible pregnant women
Newborns covered
by Lovelace or Molina Salud.
