Proctor Agreement

OBTAINING A PROCTOR

• If you live within a fifty mile radius from San Juan College, you will take your tests at San Juan College. You will arrange a time with the Health Information Technology Department.
• You will need to provide proper identification when you take an examination
• If you live outside the fifty-mile radius you will arrange for a proctor. Appropriate proctors include Directors of Health Information Programs, Instructors, Librarians, and Testing Centers located at colleges and universities. Family members and friends are not acceptable proctors. Contact The Health Information Technology Department at (505) 566-3597 if you have any questions about approved proctors
• Print out the proctor agreement pdf document
• Have your proctor sign the agreement and mail it to:

San Juan College HIT Programs
4601 College Blvd
Farmington, NM  87401 or Fax it to (505) 566-3820

• Send in the agreement at the beginning of the semester. Do not wait until an examination is due. Proctors must be approved before passwords will be released.
