Admissions Application

1. Fill out form completely.  We cannot accept incomplete forms!

2. May be faxed or mailed to us.

Change of Information
Instructions
1. Fill out form completely.
2. If you are submitting a name change, please include a copy of your drivers license.
3. May be faxed or mailed to us.

Request for Official Transcript (to complete admissions process at SJC)
Instructions
1. Fill out form completely.
2. Send to your high school or college. (they may have fees)

Request for Official SJC Transcript ($2 per copy)
Instructions
1. Fill out form completely.
2. May be faxed with payment made to Business Office by Credit Card.
3. May be mailed in with check or money order payable to San Juan College.

Graduation Application
Instructions
1. Fill out form completely.
2. Must include correct Catalog Year and your Advisor’s Signature.
3. Must attach your Degree Checklist.  (see your Advisor)
4. Return by appropriate deadline listed on application.
5. Must bring to admissions office in person.  Will not accept mailed or faxed copy.
6. If planning to participate in spring graduation ceremonies, please contact SJC Bookstore before appropriate deadline.
